New Treatment Strategies for Endometriosis and Adenomyosis

Oral GnRH antagonists have become an important advancement in the medical management of endometriosis and are increasingly being explored for adenomyosis. These agents work by directly blocking GnRH receptors in the pituitary gland, producing an immediate decrease in LH and FSH and therefore suppressing ovarian estrogen production without causing the “flare effect” seen with older GnRH agonists. Because they offer rapid onset, reversible suppression, and dose-dependent estrogen reduction, they allow clinicians to tailor treatment intensity to balance symptom relief with side-effect risk. In endometriosis, oral antagonists such as elagolix, relugolix combination therapy, and investigational agents like linzagolix have demonstrated significant reductions in dysmenorrhea, non-menstrual pelvic pain, and dyspareunia (painful intercourse) in large randomized trials. A major advantage is the flexibility of titration: lower doses achieve partial estrogen suppression with fewer hypoestrogenic symptoms, while higher doses provide stronger pain control but at the cost of greater risks such as hot flashes and bone mineral density loss. Many patients benefit from “add-back therapy,” which uses low-dose estrogen and/or progestin to mitigate bone loss and vasomotor symptoms while maintaining efficacy. Compared with older GnRH agonists, oral antagonists avoid injections, act immediately, and can be easily stopped or adjusted. However, their use still requires monitoring of bone health, vasomotor symptoms, and long-term safety, as prolonged therapy and repeat courses are areas where evidence continues to evolve.

The addition of a low-dose oral contraceptive, “add back therapy” typically containing estrogen with a progestin or progestin alone, partially restores circulating estrogen levels while maintaining suppression below the threshold that stimulates endometrial lesion growth. This approach preserves analgesic efficacy while mitigating adverse effects, improving treatment adherence, and allowing longer durations of therapy. Furthermore, the progestin component enhances decidualization of the endometrium, the lining of the uterus, and atrophy of ectopic endometrial tissue, reinforcing lesion suppression. Overall, the combination of oral GnRH antagonists with an oral contraceptive offers a flexible, reversible, and patient-centered treatment option for individuals with moderate to severe endometriosis, particularly those who require sustained symptom control and improved quality of life

The use of oral GnRH antagonists in adenomyosis is more recent and remains largely investigational. Early pilot studies and case reports—mostly involving linzagolix—show promising outcomes: reductions in uterine volume, improvement in pelvic pain and heavy menstrual bleeding, and better quality of life. Because adenomyosis is strongly estrogen-responsive, inducing a controlled hypoestrogenic state is a biologically plausible strategy, and the reversibility and dose-titration of oral antagonists make them attractive alternatives to surgery or long-term progestin therapy. Small studies have used an approach of initial “full suppression” (e.g., a higher dose for 12 weeks) followed by lower-dose maintenance therapy, demonstrating meaningful symptom improvements. Still, evidence is limited, and major guidelines have not yet adopted oral GnRH antagonists as a standard treatment for adenomyosis; larger randomized trials are needed to clarify long-term outcomes, fertility impact, adverse-effect profile, and durability of symptom relief after stopping the medication.

Overall, oral GnRH antagonists represent a flexible, convenient, and effective medical option for endometriosis and a promising—but still experimental—therapy for adenomyosis. They offer meaningful symptom control while avoiding injections and flare effects, but require careful management of hypoestrogenic risks, especially bone health. Their growing use reflects a larger shift toward personalized, titratable hormonal suppression, and ongoing research is focused on optimizing dosing, extending safe duration of therapy, and evaluating their role in long-term disease modification, fertility preservation, and uterine-sparing treatment strategies.

Summary At mid-cycle, uterine contractions are predominantly cervical-fundal (from the bottom of the uterus to the top), specifically toward the fallopian tubes. The amplitude and frequency of the contractions increase significantly as ovulation approaches. There is evidence that this facilitates sperm ascension towards the distal (far) end of the fallopian tubes, where fertilization occurs. Women with endometriosis display marked uterine hyperperistalsis that differs significantly from the contractions of women without endometriosis. At mid-cycle, uterine contractions in women with endometriosis became dysperistaltic (abnormal), arrhythmic, and convulsive, while in controls, peristalsis continues to show long and regular cervical-fundal (normal) contractions. This may explain the high incidence of infertility even in women with only mild endometriosis.
